A Provider Transaction Account Number (PTAN) is a Medicare-specific identifier that a Medicare Administrative Contractor may assign to a provider's enrollment record. It identifies the provider's enrollment with that contractor and is used together with the National Provider Identifier (NPI) on Medicare claims and correspondence, even though the NPI is the primary national identifier.
The PTAN is tied to a specific enrollment with a specific MAC, so a provider enrolled with multiple MACs or under multiple practice locations may hold more than one PTAN, while holding a single NPI. It is a Medicare administrative identifier, not a national standard.
In practice
Because the PTAN pairs with the NPI in Medicare's records, a mismatch — a PTAN that does not correspond to the NPI on the claim, or an enrollment that has lapsed — is a common source of Medicare rejections. Confirming the current PTAN-NPI pairing for each enrollment is part of Medicare billing hygiene.
