Activity or decision

A specific action, control, handoff, or decision point.

Accountable role

The one role answerable for the outcome.

Responsible role

The role completing the work; list additional roles only when necessary.

Consulted roles

Roles that provide input before the action or decision.

Informed roles

Roles notified after the action or decision.

Decision criteria

The policy, evidence, or threshold used for a decision.

Completion evidence

The system state, report, acknowledgment, or record that proves completion.

Escalation trigger

The condition or elapsed time that starts escalation.

Review owner and date

The person responsible for validating the assignment and its next review date.
