Use this session-only checklist to structure an eligibility check before a visit. It does not confirm coverage, benefits, or authorization for a specific patient — the payer's eligibility response and policy remain the controlling sources.

The check is run ahead of the visit and re-checked when neededVerify scheduled patients in advance; re-verify new patients, plan changes, and records that have gone stale since the last visit.The subscriber and member ID are correct for this patientWhen the patient is a dependent, use the subscriber's member ID and demographics with the correct relationship, not the patient's.Coverage is active on the date of serviceConfirm the plan's effective and termination dates cover the actual service date, not just that coverage is active today.The correct plan and network status were identifiedConfirm the specific plan you will bill and the provider's in- or out-of-network status for that plan.Primary and secondary coverage were determinedWhere the patient has more than one plan, confirm which pays first before the claim is built.Benefits and cost share were capturedRecord covered status, deductible met, copay, coinsurance, and any visit or unit limits from the response.Referral and prior authorization requirements were checkedIdentify any service that requires a referral or authorization so it can be obtained before the visit.The eligibility response was recorded with its date and referenceSave the response and any payer reference or trace number in the secured system as evidence of what was verified.
